How to File a Claim

  1. Make sure you have the following information:
    • Phone number for the device
    • Make and model number for the device
    • Account holder billing address
    • Date, time, location, and details on the incident
    • Credit card or debit card info for paying the deductible
  2. Call Customer Care at 1-855-309-8342 to file a claim, or File a Claim online to start the process.
  3. Once the claim is approved, pay your deductible (see chart below).
  4. Watch for your replacement phone; open and activate it promptly when it arrives.
